2011-2012 Instructions for Appeal Related to Student or Parent Disability

You may request a re-evaluation based on a change in household income if you or your parent suffers a temporary or permanent disability which results in a reduction to the household income.

You may also request reasonable modifications to financial aid policies, practices, or procedures if necessary to accommodate a student disability. You must support your request with documentation.

Disabled Student:

Documentation required includes the following:

All appeals require submission of the your and/or parents’ 2010 federal tax forms or non-filer form, a physician’s statement or other documentation of the disability and a statement providing what type of accommodation is being requested.

If the request for reevaluation is based on change in income, you must also submit the Student and/or Parent Income and Expense Form.

The Financial Aid Appeals Committee will confirm with the Director of Disability Support Services (DSS) that appropriate documentation is on file if required. If more specific documentation is needed, the DSS Office will request the information and forward it to the Financial Aid Appeals Committee.

Disabled Parent:

Documentation required: 2010 parent federal tax forms, physician’s statement or other documentation of disability and Parent Projected Year Income form.

Process:

Once all documentation has been compiled, the Financial Aid Appeals Committee will consult with the Director of DSS to ensure that the student’s request in considered in light of Section 504 standards. Once a decision regarding the appeal is made, the Financial Aid Appeals Committee will inform the student in writing. If the appeal is denied, the Financial Aid Office will include the basis of the denial in its notice to you. If you wish to contest this decision, you may file a grievance using the ‘LMU Grievance Procedures for Persons with Disabilities’.

The above-described procedures neither prevent nor substitute for a student filing a grievance with the Office for Civil Rights.
